This session will address two important and debated topics in colorectal cancer surgery. First, the role of routine lateral pelvic lymph node dissection (LLND) will be discussed, focusing on its indication for rectal cancer patients with clinically suspicious lateral pelvic nodes persisting after neoadjuvant therapy, where selective LLND may reduce local recurrence and improve outcomes. Second, the session will explore the optimal extent of colectomy in patients with Lynch syndrome and localized colorectal cancer, weighing the benefits of segmental colectomy—which preserves bowel function but carries a higher risk of metachronous cancers—against extended colectomy, which reduces future cancer risk but may impact quality of life. The discussion will highlight current evidence, guideline recommendations, and the need for individualized, multidisciplinary decision-making in these complex clinical scenarios.
